Minister in the Presidency Manto Tshabala-Msimang is reportedly gravely ill and might have to undergo another liver transplant.
The City Press newspaper reported her body was rejecting the liver she received from a teenage suicide victim two years ago.
The paper said that the minister had been hospitalised after there were apparent complications with her first transplanted liver.
Her physician listed her condition as stable but said she had been in the Charlotte Maxeke Hospital for the past two weeks.
The former Health minister had apparently been told by her doctors to prepare for a second transplant operation but that would have to be approved by a panel of specialists and there has to be a strong motivation for the procedure.
